All units went en route and Battalion-41 moved all traffic to tactical c h annel one. Engine 43 and B-41 arrived o n scene to find a large column of smoke with flames above the roof line on t h e bravo charl i e side of the structure. B-41 met family at the street and was advised all persons were clear of the structure. B-41 performed a 360 of the structure and gave a size up of a two story residential structure , wood frame brick veneer with heavy flames on the bravo c h arlie corner of the stru cture . B-41 returned to the command vehic l e and advised E-43 and Engine 42 to pull 2 1/2 lines to the front door and the rear of the structure. One skid load had been pul l ed from E-43 and command advised that we would be operating with 2 1/2 inch lines. Bystanders and family members of the home advised that there were still animals in the home. 8119 had arrived after B-41 and reported that they would possibly transport three members to the hospital. B-41 requested an additional Rural Metro unit. The initial plan was to make entry on the alpha side of the structure and attempt to push the fire back from the front and hold the fire off the delta side of the structure . E-41 crew on scene and the Engine was advised to report to the hydrant on White Columns Dr . and remaining crew members to the front door to assist in establishing another 2 1/2 inch line from E-41. 872 arrived on scene a n d was assigned to safety. E-43 was parked on the hydrant on White Columns Ct. Crews pulled l ines to the front door from E-43 and were met with flames coming t h rough the door and out the window above the front door and pushed back from the heat . Heat from the fire was busting windows across the front of the home. E-42 reported that there was fire in the basement area and all crews on scene were advised that this would be a defensive attack until we could k nock the bul k of the fire down. One 2 1/2 inch line was flowed from the front door and the other was flowed through the rear of the structure. Milton T-1 advised that they were three minutes out and asked for an assignment., B-41 advised T-1 that we would spot T-1 on the alpha side of the structure and set up to flow the ladder pipe from the street to try and knock the fire down and would be supplied by E-41. E-4 3 ended up supplying T-1 for the operation and met resistance in trying to supply the hand l ines and the ladder pipe at the same time. B-41 called for 2 1/2 hand lines to be pulled from E-41 to alleviate the demand for water on the plug that E-4 3 was on. The rapid attack monitor was pulled from T-1 and placed on the Charlie side of the structure.B-4 1 checked with radio on the request for gas and power and radio advised that they had not been requested but would be summoned. Sawnee power advised that they had an extended ETA. Gas company arrived within 30 minutes of our request. E-43 crew met with Gas company and was s hu tting the line down as the gas company reported to the meter to shu t it off. Lines were flowed until the fire was knocked back to a point where crews could start maki n g limited entry into the structure to extinguish remaining hot spots. Command requested B-1 to start an Alpharetta unit to the scene and Alpharetta Engine 4 was summoned. Upon their arrival they were ut ilized to rotate Milton crews out to rehab. Crews cont inued working on hot spots and all crews were rotated through rehab for one rotation. 870 , 871 , 873 on scene assisting in overseeing fireground operations. 873 began investigations and ended up focusing on the bravo charlie corner of the structure , once 873 released crews after his investigation and final mop up was performed of the structure . 1-800-Board Up responded to the scene and provided bags for the animals (two cats and two dogs) t h at were located to be placed in. Late in the operation Board Up began covering openings in preparation of securing the structure. Milton crews and Board Up removed valuables throughout the operation to the front yard. Some items were taken to the owners and others were left secured in the structure after Board Up finished their operation . Fire watch was conducted throughout the eve n ing with no visible signs of smoke n oted . Scene was left i n Board Ups possession and we were notified when they cleared the scene. This call occurred at shift change and FF Clausen , Sprouse , Seay , Womack and FAO Murray and Capt. James responded to the scene on frontline equipment with the C shift, and as operations allowed these members were pulled from the scene and returned to the fire station for relief from duty. Observations After assisting with Suppression operations at the fire scene, I began an investigation into the origin and cause . The structure was a large, single -family two story on a basement Type VB structure. The 4376 square foot structure was located on a 1 acre lot in the White Columns subdivision. Both physical and testimonial evidence pointed to an origin in the area of the BC corner. In this area of the home -which was one story on a basement here -the floor had collapsed entirely, with the fireplace and keeping room accoutrements falling into the basement . Additionally, the roof and attic was almost completely burned away at this part of the house. An investigation of the rest of the house revealed that the structural members were largely intact the farther away from this area the investigation went, with relatively clean studs still found throughout the house (though contents were largely destroyed). Upon speaking with the family, I learned that no one smoked and there were no noticeable issues with the electrical system , nor was there any recent work on the house . The residents of the house included Seven and Dawn Crain (parents) and 7 juveniles (aged 16, 14, 12, 9, 7, 7, and 3). The family was in the middle of their morning routine when the fire occurred . Both a computer and coffee maker were used in the morning. While family members were in and out throughout the morning, the 9 year old daughter and two 7 year olds were in the kitchen and keeping room area at the time of the fire . The daughter advised that the brothers were in the other room, she smelled plastic burning. She also advised that there was a red lighter in that room. When the boys were asked about the fire, they stated that they saw the fire and called 911. When I asked them "how would someone start a fire?" I was told : "Someone would push the button and light the table ." However, when asked directly if they started the fire, they denied it. During the interviews, I also learned that someone had tried to light a fire on the previous evening . Apparently the gas was turned on, but they couldn't light the fire, and that there was a gas smell. Apparently the family went to bed without further issue. Conclusions The cause of the fire is accidental. The fire's origin was in the area of the keeping room on the BC corner of the house . The testimonial evidence of both the attempted use of the fireplace the previous evening and the juveniles with lighters tend to support an accidental cause associated with the fireplace. Upon entering the subdivision behind Engine 42 and Rescue 42 dispatch advised that AMR was on scene and advised fire was coming from the chimney. E-42 arrived on scene with a size up of the same that there was fire showing from the top of the chimney. E-42 passed the structure on the D side and stopped their apparatus. B-4 stopped sho11 of the scene and E-42 started a 360 advising they would get a ladder to the C side of the structure and attempt to extinguish the fire . Battalion 4 assumed command and advised E-42 to pull the skid to the front door in preparation to make entry into the attic , an orange glow and fire could be seen in the attic with active fire outside of the chimney chase. Dispatch was advised to make a senior staff notification. E-42 deployed the skid load to the door, and rescue 42 was advised to shut off utilities and report to command. The crew of E-43 and E-42 was split to make an entry team and a RIT team and E-42 crew made entry progressing to the attic. T-41 arrived and E-85. E-85 was assigned to safety and T-41 deployed a hose line to the rear of the structure in attempt to hold the fire in check until the interior team could access the attic. E-42 crew was able to access the second floor and knock the fire down utilizing the attic access ladder. After advising that the fire was knocked down, T-41 advised that the exterior chase was still burning, and crews worked to get water up to the ridge line both interior and exterior. Once crews had the fire knocked down they retreated to the front yard for rehab. Alpharetta Battalion 8 arrived along with 401 and both the Fire Marshal and Assistant Fire Marshal for Milton. After letting the house sit, smoke began to show from the chase at the ridge line on the C side of the structure again and crews worked from both the inside and exterior to extinguish. Once the chase was extinguished, crews checked the structure from top to bottom with the Thermal Imaging camera with no fire or heat noted. Fire damage was confined to the ridge of the attic through the full length of the house and the fireplace chase from the second floor to the roof. Water damage was throughout the home and tarps were placed on the first floor to cover items that could be salvaged . B-4 made contact with the homeowner on several occasions during the fire and they advised that they had a fire in the ground floor fireplace throughout the day and that a passerby noticed fire at the top of the chase exterior. The homeowner opened the attic access and saw flames at the top of the chase near the ridge line and attempted to discharge their extinguisher with no success at getting the fire out. The homeowners were in the yard upon our arrival and visibly upset. Milton PD assisted in moving everyone to the opposite side of the street and out of harm 's way. At the completion of our operations, I advised the homeowner that we would return around 0200 hours for fire watch and that we would leave the rear French door at the pool unlocked for access. The homeowner advised that would be fine and that they would be staying next door for the night. Investigator's Initial Observations I (AFM Alex Fortner) arrived on scene at 305 White Columns Court, Milton , Georgia at approximately 2000 hours on 0 1/08/2018 to conduct a fire investigation per Battalion 4. Upon an-ival , I was briefed by Command (Battalion 4) and Deputy Chief Marietta on the timeline of events before my arrival. From the exterior, conditions looked favorable (with the exception of fire suppression equipment and personnel on scene) with only minor/moderate smoke visible from the chimney cap and the peak of the roof. This same property/owners had a previous fire on 11/22/2013. See report number 13-0001744 for details. Investigation While overhaul and smoke ejection efforts continued inside by fire service personnel, I spoke with the homeowners on the events before calling 911. In a written statement, Mrs . Cain stated, "Made a fire in FP around 1pm. Kept it low throughout the day. 7:05 someone rang door and said he saw flames on root1/chimney. I ran and called 911 at 7:08. My husband Steve, put fire in FP out with a panful of H2O . Son, John ran upstairs with husband and pulled down ladder stairs to attic . I ran into kitchen and grabbed 2 fire extinguishers. They observed that there was NO fire in attic at that time. Husband and son said it was up the flue . They aimed both extinguishers at the flue. Could see light and em bers failing down . Kept waiting for the fire department to get here. We told them exactly where the fire was coming from. Seemed like it took a very long time to get water into house and up chimney." Mr . Cain told a similar story verbally. Mr. and Mrs . Cain seemed distraught and couldn't believe that their hou se had caught fire again. After overhaul efforts halted , an initial investigation survey of the incident began. Interior and exterior photos were obtained, as well as anything suspect of relevance. In general, the fire damage was noted as heavy within the attic space. Although smoke was seen in various areas throughout the house , it was noted as light and minimal. Water damage was noted as heavy throughout the entire structure. A further in-depth investigation ensued to locate the exact area of origin and cause . Additional photos were ta ken of the attic and chimney chase (where the most extensive fire damage was located). The homeowners seemed genuinely concerned and distraught by the fire and its overall damage. Area of Origin/Cause Based on my observations , presented evidence, first-arri ving fire crew's testimony , and oral/written witness statements, the fire origin was determined to be in the uppermost part of the chimney chase within the attic space. The chimney chase housed two flues; one flue was for the first floor fireplace and the other flue was for the basement fireplace. The basement fireplace flue damper was closed and there was also no signs of heating/bmning . The first floor fireplace had been burning slowly for several hours befo re the fire incident, and was not overloaded with wood. According to the homeowners , the entire fireplace system was newly installed after the destruction from the previous fire. According to the homeowners, since moving back into the house , the newly installed fireplace system has been used carefully and only a handful of times (because of their understandable fear of fire). The cause of the fire will be labeled as accidental in nature and attributed to the failure of the first floor fireplace flue. The area of origin and newly constructed fireplace components points to a possible installation en-or of the flue itself. There were no other heat/ignition source noted in the area of origin , and there has been no repair work done on the house since the remodel. Since the fireplace system is new and rarely used , the flue had not been cleaned since its recent installation. There were no signs of an intentionally set fire. Nothing else was fow1d at the scene to be notable . Upon entering the subdivision behind Engine 42 and Rescue 42 dispatch advised that AMR was on scene and advised fire was coming from the chimney. E-42 arrived on scene with a size up of the same that there was fire showing from the top of the chimney. E-42 passed the structure on the D side and stopped their apparatus B -4 stopped short of the scene and E-42 started a 360 advising they would get a ladder to the C side of the structure and attempt to extinguish the fire. Battalion 4 assumed command and advised E-42 to pull the skid to the front door in preparation to make entry into the attic, an orange glow and fire could be seen in the attic with active fire outside of the chimney chase . Dispatch was advised to make a senior staff notification. E-42 deployed the skid load to the door, and rescue 42 was advised to shut off utilities and report to command. The crew of E-43 and E-42 was split to make an entry team and a RIT team and E-42 crew made entry progressing to the attic. T-41 arrived and E -85. E -85 was assigned to safety and T -41 deployed a hoseline to the rear of the structure in attempt to hold the fire in check until the interior team could access the attic. E-42 crew was able to access the second floor and knock the fire down utilizing the attic access ladder. After advising that the fire was knocked down, T-41 advised that the exterior chase was still burning, and crews worked to get water up to the ridge line both interior and exterior. Water damage was throughout the home and tarps were placed on the first floor to cover items that could be salvaged. B -4 made contact with the homeowner on several occasions during the fire and they advised that they had a fire in the ground floor fireplace throughout the day and that a passerby noticed fire at the top of the chase exterior . The homeowner opened the att i c access and saw flames at the top of the chase near the ridge line and attempted to discharge their extinguisher with no success at getting the fire out . The homeowners were in the yard upon our arrival and visibly upset. Milton PD assisted in moving everyone to the opposite side of the street and out of harms way . At the complet i on of our operations, I advised the homeowner that we woul d return around 0200 hours for fire watch and that we would leave the rear french door at the pool unlocked for access. Upon arrival, E42 noted fire coming from the chimney of a two story, wood frame residence . E42 officer initially establ ished White Co lumns Command, but B4 arrive d moments later and took Command. E42 FF was advised to pull the rear skid load to the front door while the officer performed a 360 of the property. E42 driver positioned the apparatus on the D side of the structure, and then obtained positive water supply with the hydrant by the Eng i ne. After the 360, E42 FF was advised to begin to hit the roof with a fire attack from the exterior. E42 officer and E43 officer went inside and advised Command that there was visible fire in the attic. E42 Officer, FF, and E43 AO and FF pulled the line into attic and put an initial knock down on the fire in the attic as "fire attack" crew . "Fire attack" crew hit the fire in the attic and also the fireplace chase , getting a knock down on the fire. E42 was then assigned to search for hot spots in the a t tic , first floor chimney chase, and second floor chimney chase. E42 crew pulled sheet rock on the first floor to expose potential hot spots. The E42 crew worked at salvage, placing tarps over property on the first floor. E42 crew and E43 crew later performed overhaul in the attic, trying to expose any remaining hot spots in the chimney chase . Upon arrival, I was briefed by Command {Battalion 4) and Deputy Chief Marietta on the timeline of events before my arrival. From the exterior, conditions looked favorable (with the exception of fire suppression equipment and personnel on scene) with only minor/moderate smoke visible from the chimney cap and the peak of the roof. This same property/owners had a previous fire on 11/22 /20 13. See report number 13-0001744 for details. Investigat ion While overhaul and smoke ejection efforts continued inside by fire service personnel, I spoke with the homeowners on the events before calling 911 . In a written statement, Mrs. Cain stated, "Made a f ire in FP around 1pm. Kept it low throughout the day. 7:05 someone rang door and said he saw flames on roof/chimney. I ran and cal l ed 911 at 7:08. My husband Steve, put fire in FP out wi t h a panful of H2O. Son, John ran upstairs with husband and pulled down ladder stairs to attic. I ran into kitchen and grabbed 2 fire extinguishers . They observed that there was NO fire in attic at that time . Husband and son said it was up the flue. They aimed both extinguishers at the flue. Could see light and embers failing down. Kept waiting for the fire department to get here. We told them exactly where the fire was coming from. Seemed like it took a very long time to get water into house and up chimney ." Mr. Cain told a similar story verbally . Mr. and Mrs. Cain seemed distraught and couldn't believe that their house had caught fire again. After overhaul efforts halted, an initial inve stigation survey of the incident began. Interior and exterior photos were obtained, as well as anything suspect of relevance. In general, the fire damage was noted as heavy within the attic space. Although smoke was seen in various areas throughout the house, it was noted as light and minimal. Water damage was noted as heavy throughout the entire structur e . A further in-depth investigation ensued to locate the exact area of origin and cause. Additional photos were taken of the attic and chimney chas e (where the most extensive fire damage was l ocat ed ). The homeowne rs seemed genuinely concerned and distraught by the fire and its overall damage . Area of Origin/Cause Based on my observations, presented evidence, first-arriving fire crew's testimony, and oral /written wi tness statements , the fire ori gin was determined to be in the uppermost part o f the chimney chase within the attic space . The chimney chase housed two flues; one flue was for t he fi rst floor fireplace and the other flue was for the basement fireplace. The basement fireplace flue damper was closed and the re was a l so no signs of heating /burning. The first floor fireplace had been burning slowly for several hours before the fire incident, and was not o ve rloaded wi th wood . According to the homeowners, the entire fireplace system was newly installed after the destruction from the p r evious f ire. According to the homeowners, since mo v ing back into the house, the newly installed fireplace system has been used carefully and only a handful of times (because of their understandable fear of fire). The area of origin and newly constructed fireplace components points to a possible installation error of the flue itself. There were no other heat/ignition source noted in the area of origin, and there has been no repair work done on the house since the remodel. Since the fireplace system is new and rarely used, the flue had not been cleaned since its recent installation. There were no signs of an intentionally set fire. Nothing else was found at the scene to be notable . The investigation was complete at approximately 2300 hours.
