A motion to approve the meeting agenda was made by Mr. Cranmer and
seconded by Mr. Shannon. The vote passed unanimously.
A motion was made by Mr. Costanzo and seconded by Mr. Alberici to approve
the January 2019 minutes. The vote passed unanimously.
PUBLIC COMMENT
None
1. Hopewell Youth Association
• Tom McKlveen and HYA are working on getting batting cages and
parking set up at BUMC Field.
• There are over 680 participants and 70 teams which is a 20%
increase from last spring.
• HYA is requesting an indoor lighted batting facility.
2. Eagle Stix
o Excessive rain has created flooding conditions at Northwestern
Middle School. Eagle Stix would like access to the artificial turf
fields.

UNFINISHED BUSINESS
1. Discussion of Comprehensive Master Plan for Parks and Recreation
o A presentation was delivered by Mr. Steve Provost of Barge Design
Solutions.
o Mr. Scott Sawyer spoke and offered several constructive comments
regarding recently acquired land and other greenspace bond
issues.
o Ms. Judy Burds expressed her satisfaction with the direction the
report takes in regard to Birmingham Park. She also asked for a Dog
Park.
o A motion to accept the report was made by Mr. Shannon and was
seconded by Mr. Costanzo. The motion passed by a vote of 6 - 0 -
1 with Mr. Hill abstaining.
NEW BUSINESS
1. Discussion of a proposed Financial Aid Program
o Mr. McKlveen distributed documents related to an idea he is
working on to create a financial aid program to assist families in
need to participate in the City of Milton Parks and Recreation
programs.
o The plan was reviewed by the PRAB members and it was agreed
that they will review it at the next meeting.

A motion was made by Mr. Alberici and seconded by Mr. Shannon to approve
the December 2018 minutes. The vote passed unanimously.

1. Hopewell Youth Association
o Early and regular registration has ended. Late registration is still
taking registrations. This season will have high participation. They are
moving forward.

2. Eagle Stix
o Registration and planning are in full swing. The season begins in a
couple weeks. There will be a coaches meeting on 1 /26 in
Community Place. Spring registration numbers are not complete
yet.
3. Milton Steelers
o The Milton Steelers and the City of Milton have parted ways.

NEW BUSINESS
1. Discussion of MOU
o Jay Beta- Mr, Beta is concerned about gymnastics. Currently, his
annual cost has gone up by $1,000. If his child were to go to
competitive, it would be $1,300-1,400 more. If the city wants to start
a gym without proper equipment, don't start at all.
o Megan Small- Mrs. Small is a mother of four with all her children
taking part in classes at the Alpharetta Recreation Center. In total
her children take part in eight classes. She pays approximately $300
per child and without the MOU her fees will increase 75%. She
understands the need to build for the future but is concerned with
how her family will navigate the present with the increase in fees.
2. Comprehensive City -Wide Master Plan
o Jim Cregge PRAB will vote on the new Master Plan next meeting.
This plan will create the guide book of where we are at and where
we need to go. With a favorable vote from the board, it will be
presented to Council.

3. New Program Offerings
o Jim Cregge- we are looking to change the typical model and
going to businesses in Milton or outside Milton to partner with.
Businesses based in Milton are given priority over non -Milton
businesses.
o Jason Alberici- Stated this model worked well for him in his past. He's
had experience with this sort of model.
o Scott Stachowski- He mentioned that the trick is to find the popular
programs. Do we branch out to offer this to competing programs
under the same sport? All members feel it is something to pursue.
4. New Employee
o Ryan Flack- Ryan has started an 'internship with the City of Milton.
He's a student at the University of Mississippi and working towards
finishing a degree in Recreational Administration.
CITY STAFF REPORTS AND COMMUNICATIONS
1. Jim Cregge
o Shade structure project - concerns about bid responses - rebidding
the project.
o Lacrosse wall -will be ready forbid in the next couple of weeks.
o BUMC Field - Agreed on the lease of the field. Work is ongoing.
o Milton Steelers Officially parted ways. Things have worked out for
both parties. RFP for football has been released.

Financial Aid Program
POLICY STATEMENT:
The City of Milton has approved a policy to provide a Financial Aid Program for
qualified economically -challenged families to enroll their children in a variety of leisure
activities offered by the City of Milton Parks and Recreation Department ("City"). To be
considered for Financial Aid, a parent or guardian must complete the application form
and attach all required documentation for each participant. The criteria for eligibility
and the application process are listed below.
CRITERIA FOR ELIGIBILITY:
The criteria for eligibility in the financial aid program are as follows:
1. The applicant must reside within the taxpaying boundaries of the City of Milton, Georgia.
2. The applicant's child/children must qualify for the Fulton County School lunch
program (full or reduced).
3. If the applicant's child/children are not involved in the Fulton County School
lunch program, the applicant's TOTAL monthly household income must meet
current Federal Poverty Guidelines. The City reserves the right to adjust income
guidelines or reduce the allotted amount per child as deemed necessary and
without notice.
4. The Financial Aid Program applies only to school -age children (age 18 and
under). Adults are not eligible to enroll in programs under the financial aid
program.

APPLICATION PROCESS:
The Department Director shall designate an employee to oversee the Financial Aid
Program. The employee is responsible to review all applications and required
documentation, determine whether or not applicants meet the City's criteria for
eligibility, and either approve or deny the application for financial aid based on the
information presented by the applicant. The employee is responsible to maintain
accurate records of applications, approvals, denials, and lists of approved
applications. Employees involved in the financial aid application process are
responsible to maintain the confidentiality of the applicants and store information
provided by applicants in a secure location.
The Financial Aid applicant is responsible to complete and submit all required
documentation to assigned employee and/or facility at least five (5) business days
prior to the end date of registration or before the requested program begins.
Incomplete applications will not be considered and will be returned promptly to the
applicant for corrections.
Assigned employee is responsible to schedule meetings with financial aid program
applicants to review and verify required documentation including but not limited to the
following: residency verification, hardship affidavit, income verification, dependency
verification, verification of participation in the Fulton County School lunch program, etc.
Employee is responsible to communicate findings and decision with the financial aid
applicant within three (3) business days after the meeting, provided all documentation
was presented to the employee during the meeting. If additional information is
requested of the applicant, the employee shall allow the applicant ample time to
obtain additional documentation.
Once a decision on the application has been made, the employee will sign and date
the application where indicated, noting whether or not the application was approved
or denied. If approved, the employee will note the expiration date on the application.
If denied, the employee will note the reason for denial of the application. This
information will be provided in writing (via email or U.S. mail) to the applicant. The
employee shall maintain a list of active financial aid recipients and shall provide this list
to other City staff as deemed appropriate for others to carry out their job duties.
If an applicant is approved, the employee will notify the program provider of financial
aid approval. Space in programs is not guaranteed by financial aid approval. Supply
fees, equipment fees, trainer fees, family pool passes, etc. are not included in the
financial aid program.
Applicants may be approved for a maximum allowance of 2 activities per child per six
(6)month approval period. Applicants must re -apply for financial aid program every six
(6) months.
Misuse or abuse of this privilege as determined by the City may result in temporary
and/or permanent suspension of financial aid eligibility, and removal from the
recreation program(s) pertaining to financial aid. Examples of misuse or abuse may
include but are not limited to: failure to show up for a program without advance
notification to the program provider, failure to pay program supply fee (if applicable),
misrepresentation of income and/or residency, etc.
Hardship affidavits are executed under penalty of perjury. Providing false affidavit can
result in criminal consequences, including possible incarceration. If it is discovered that
the information provided by the applicant was altered, false, fabricated, or otherwise
misleading or untrue, applicant is responsible for reimbursement of any financial aid
funds received and is subject to all applicable criminal and civil claims.
WHAT DOCUMENTATION IS REQUIRED?
1) Eligibility Verification - If applicant's child/children qualify for the Fulton County School lunch
program (full or reduced), applicant must provide a statement/letter/card from Fulton County
Schools.
2) Residency Verification - Applicant must supply proof of residency by submitting one of the
following with the application:
a) Current letter from Fulton County Housing Authority
b) City of Milton property tax receipt
c) Current City of Milton utility or sanitation bill with applicant's name
d) Current rental lease that lists names of individuals living in household
e) Current letter from a state or federally funded agency
f) Current Section 8 letter
3) Income Verification - Applicant must supply verification of income by presenting one of the
following with application:
a) Current income tax return
b) Current W2 forms
c) Current pay stubs) - one month
4) In addition to one of the above, applicants may submit documentation such as a current
statement/letter/card from a local, state or federally funded agency including:
a) TANF card
b) Section 8
c) DFCS
d) Social Security
e) INS
f) Unemployment Office
5) Dependency Verification - Applicant must supply proof of dependents by presenting one of
the following with application:
a) Birth Certificate
b) Certificate of Adoption (or other official order of the court)
c) Letter of Guardianship (or other official order of the court)
6) Hardship Affidavit - Applicant must submit a notarized hardship affidavit with application
a) Hardship Affidavits are executed under penalty of perjury. Providing false affidavit can
result in criminal consequences, including possible incarceration.
7) Other- If documentation fisted in items A, B, C, and D is not available, applicant may submit
alternative documentation for consideration.
If it is discovered that the information provided by the applicant was altered, false,
fabricated, or otherwise misleading or untrue,. applicant is responsible for
reimbursement of any funds received and is subject to all applicable criminal and civil
claims.
